Sounds like fissure: I would recommend fiber supplementation to bulk and soften stools, sits water baths. If continues, see your doctor. Some medications can help but avoid otc creams. Surgery is gold standard but most patients leave that as last resort. Other more rare diagnoses are possible.

Sounds like a : Fissure, or tear in the anal canal, tearing sensation with bm and post evacuation pulsating throbbing pain. The dizzies and nausea are a vagal response to pain. Try increasing your fluid intake to 80-100 oz of caffeine free/alcohol free fluid perm day, avoid spicy food and acidic foods such as all tomato products. Warm tub soaks after a bm plain warm water for 15-20 minutes will help.
