CHF: A patient with COPD and CHF can generally use albuterol. Some patients with CHF have signficant arrhythmias and for these there is some danger of triggering a serious arrhythmia. This discussion should be had with the patient's dr.

Alternative? : CPAP has shown great promise. Data have showed that once the CHF is treated, albuterol actually help. The worry is it might cause arrhythmia... Therefore, CPAP treatment may be a better option...

YES: If CHF is, stable, compensated and albuterol has to be used yes. There are alternatives, to try and stabilize COPD with maintenance medications so one doesn't need to use rescue albuterol too often.(inhaled steroids, laba's).
