Is there a way to determine if i have turf toe vs a fracture without an x-ray and either way does the treatment vary?

Not with 100% surety: It might be fair to assume it is turf toe if the mechanism of injury fits. Turf toe is the result of hyperextension of the big toe--like if you fall forward and your toe stays flat on the ground, or "turf". Most of the pain will be on the bottom of the foot, in the ball--it is a bad sprain or possibly a rupture of the plantar plate. An x ray would still be recommended. Immobilization for both.

NO: Furf toe could involve capsul, ligament, and fracture, usualy MRI is the standard due to soft tissue injury, on xray you can't see soft tissue injury.
