Likely 2 diff things: The regions of the brain (neuroanatomy) implicated in add are located relatively deep in the brain. Depending on the type of head trauma, its in the realm of possibility that add can be exacerbated by brain trauma. However, certain types of head trauma can lead to mild traumatic brain injury (mtbi) symptoms of which can mimic symptoms of add.
