Gender Reassignment: There are various degrees of gender reassignment. She will not be able to inseminate you, nor will you be able to have a child of mixed genes between the two of you. Outwardly she may appear to be a man but her genes will always be a woman's. If she chooses to keep her female genital (vagina, uterus, ovaries)trans she too will be able to conceive, but again with a male sperm donor.

Yes if retain uterus: If your girlfriend undergoes conventional sex change she will not be able to conceive. If however, you retain a normal physiologic ovary, uterus & vagina, it is possible for you to bear children using a genetic male's sperm or via insemination or ivf. Surrogacy or adoption are other alternatives. If you wish to use your girlfriends eggs, consider harvesting & storage prior to removal of her ovarie.

Adoption or...: Great question. Even though your mate is going to be a male, she will not have 'sperm' to mate with your egg. Thus, no natural birth is possible between you. However, one may be able to secure an outside 'sperm donor' to assist in the fertilization process. Best to discuss all this with your family physician who can direct you to the right specialists.

But as a f-m transman, there is no functioning penis and no testicles to produce sperm, so no babies are possible without a sperm donor. "sex change" is not an operation, but a complex hormonal, psychological, and social process that often involves surgical gender reassignment surgery, but no sperm-producing penis.
