Late menses: The most likely cause of delayed menses outside of pregnancy is hormonal imbalance. If you don't want to conceive i would suggest starting bcp's.

Possibilities: Could be due to an abnormal ovarian cyst that delayed onset of menses; could be due to later than normal ovulation, allowing for thicker endometrial production and menses may be preparing to start. In someone who is 48 yrs old, it is not uncommon to have irregular (lengthening intervals) due to less predictable ovulation. If pain worsens or menses doesn't start, u may want to see gyn: labs/sono.
