Ectopic pregnancy: Any lower abdomnal or pelvic pain in early pregnancy could be a sign of an abnormally located pregnancy called ectopic pregnancy, where the pregnancy is not located inside the uterine cavity. It is potentially life threatening. You should see your doctor as soon as possible for an evaluation, pelvic exam and ultrasound to make sure everything is ok... Good luck...

Tough to say: At 4 weeks gestation the likelihood that the discomfort is due to the pregnancy is small. It could be muscular esp. If reproducible. Sometimes women can have fibroids and not experience discomfort until pregnancy. The two most concerning possibilities are appendicitis and ectopic. Both conditions require evaluation by a doctor--pursue f/u if pain persists or worsens.
