Many causes: Many causes to this including a polyp on the cervix or the uterus, medication that may make it take longer for blood to clot or affect your hormones, soy, stress, other hormones issues such as from the thyroid. I would recommend you have an exam and possibly an ultrasound, and have someone review your medications.
