A good doctor should: In his mind have a differential diagnosis of what could be wrong based on a good history and clinical exam. Sometimes, further diagnostic or blood testing needs to be done. Make sure you see a hand surgeon, in the case you are seeing an internist and he is not ale to help you.

Is there an injury?: I think we need more history to help you. If there was an injury you probably have a soft tissue problem that will not show up on xray. I am not a hand surgeon, but if you have not seen one, i suggest you get an apointment. There could br a partial tendon injury, particularly if you have lost motion or strength. Good luck.
