Effexor (venlafaxine) Ineffective?: It depends upon: what it was prescribed for, how long you've been on it, and how big a dose you're on. If given for anxiety and depression, those disorders should resolve or significantly improve. Of course, it is necessary to be at the right dose for the right amount of time. These medications can take weeks (even 6-8+!) to work.

You don't improve: If your med is not working, you won't notice an improvement in how you feel. We measure this with target symptoms. These are the symptoms that you and your doctor identified as the things that were wrong before you started treatment. Think back on what those problems were, then assess whether any of those problems have improved. Sometimes you'll see the medicine has done more than you think.
