Many things: Top 3 things it can be: 1) poor circulation in your legs or pad (peripheral arterial disease) usually determined by checking the pulses in the feet. 2) a pinched nerve in your back which is seen on mri 3) neuropathy which can be caused by diabetes, chemotherapy, certain prescription medications.

Leg pain: Common causes for "extreme" leg pain include: vascular claudication from poor circulation to the extremity or neurogenic claudication from nerves compressed in the spine that causes severe pain to radiate into the legs. You could have neuropathy where damaged nerves cause burning pain. This neuropathy has many causes including diabetes. There is much more... I hope you get better.
