Seborrhea: Not all dandruff is equal. In some cases otc brands like h&s or t-gel are enough. Patients will often have to see their doctor to get something more powerful which is usually a steroid lotion or shampoo that is used carefully several times a week. I would suggest that you still use otc varieties but allow the shampoo to remain on your scalp for more than 2-3 minutes. Try applying before showering.

Severe Dandruff: Refractory dandruff can be a clue that there is an underlying skin condition that requires more aggressive treatment than is available from the usual otc dandruff shampoos that you have already tried. You might consider making an appointment with a dermatologist to review your options.
