It takes all types: Unfortunately, bras come in smaller sizes for a reason. Not everyone (even w/in a family) develops large breasts. Pregnancy or surgery are only two reliable mechanisms for breast enlargement in an adult. May suggest she consult a plastic surgeon if it is a significant issue for her.

Implants?: There are multiple risks and benefits for every surgery and breast augmentation is no different. Consult with a board certified plastic surgeon to review your sister's specific concerns. Also, realize that you will most likely need additional surgery in the future to deal with the implants.

Many reasons: The first and most common reason is that this is how much breast tissue she is born with. Nothing is more variable that the size and shape of the breasts in humans. This being said, there are other less frequent reasons that breasts do not develop on one side or both. Lack of hormonal stimulation, congenital disorder, etc.
