Tight foreskin: He is probably uncircumcised and foreskin is tight (phimosis). This might respond to application of prescription strength steroid cream. However definitive treatment for phimosis is circumcision or a less radical dorsal slit prepucioplasty, where tight area of foreskin is divided vertically & then sutured transversely. This loosens the foreskin.
