Gravity and osmosis: Distribution of excess fluid in nephrotic syndrome is throughout the body due to loss of blood proteins that hold water in the vascular system. Osmosis then drags water to areas of higher concentration. It may show more in the face because one can notice that most easily, but is generally throughout the body. This is known as anasarca. In chf, the osmotic effect is not present, so gravity wins.
