Spinal tap 5 days ago. laid flat for 48 hrs. drank caffeine. headache even lying down, dizzy standing. will go away on own or need blood patch & when?

Will resolve in most: Post dural puncture headache is a common sequelae of a lumbar puncture. The blood patch can provide immediate symptom relief from spinal headache and repair of the CSF leak, which is the basic mechanism of post dural puncture headache. The technique has a success rate of 70–98% if carried out more than 24 h after the dural puncture. I would advise that you meet with an anesthesiologist asap.

Spinal headache?: Although headaches from spinal taps can improve on their own, i prefer treating them earlier rather than waiting. The needles used to drain spinal fluid are usually larger and if several attempts were made the opening they leave can be difficult to self seal. Of concern is if the reason for the tap is fever of unknown origin. In this case there is a risk of transferring the infection.

Not typical: To have a headache even lying down if its due to CSF leak. Why did you get the tap? You need to be evaluated though to make the correct diagnosis.
