Pain: Doctors take care of thousands of patients in their careers. They learn to recognize and anticipate the patient's symptoms and responses. They aren't lying any more than a female gynecologist who's never had a baby can tell you it will hurt. They are trying to reassure and prepare you so you won't think something's wrong , or be surprised by a sudden discomfort. It's all in the service of helping.

Intention: Most male -- and female! -- gynecologists do not intend to "lie" to you or other patients. You're right that they don't know exactly how things feel to you. Sharing gender does not confer accurate empathy, because your sensations & sensitivities may differ. Still, most doctors do try to hear you. They also know when most patients feel pain, and try to care for you despite unavoidable discomfort.
