With great care: Cloxacillin, ciprofloxin and especially Levaquin (levofloxacin) have been associated with elevations in the inr in some people on coumadin/warfarin, Dicloxacillin has been reported to lower the inr. The bottom line is many medications have the potential to affect the inr. If a medication change of any kind is to occur tlose monitoring should be donehe prescribing physician needs to be aware. Drug drug interactio: Yes. But you will need to have your levels of anticoagulation followed more frequently with this combination. Make sure that you have a plan with your clinician for this monitoring.
