Hormonal fluctuation: Breakthrough bleeding may be caused by abnormally thick lining of the uterus (endometrium), pregnancy, the hormonal effects of ovulation. Smokers on hormonal birth control are at higher risk. Estrogen from ocp (pill), can cause breakthrough bleeding. Chronic breakthrough bleeding is actually the opposite, atrophic (thin) endometrium. I would suggest seeing your doc for labs and exam.
