Long-term hyperglycemia causes many of the long-term health problems including heart disease, eye, kidney, and nerve damage. The most common cause of hyperglycemia is diabetes. When diabetes is the cause, physicians typically recommend an anti-diabetic medication as treatment.

It depends: There are varying degrees of high blood sugar. When it is moderately high, you may feel fine other than some weakness etc and more frequent urination. If it is very high it can lead to a condition called diabetic ketoacidosis where can cause dehydration, kidney failure and death if not treated in a timely manner.
