No clue: You need to see your pcp and probably get a referral to a dermatologist. Amphetamines can cause reduction of blood flow to the skin (and other organs), and that may be part of what is going on, but I have not seen or heard of this complaint. I have taken care of lots and lots of kids on high doses of adderal or concerta, and have not seen this.
