He : He should definitely follow up with his doctor for re-evaluation. In the meantime in addition to the suppository, he should be using a rectal cream applied directly to the fissure. Any over the counter hemorrhoid cream containing Hydrocortisone should help. He should use it four times a day. Also Ibuprofen or tylenol (acetaminophen) can help ease the pain while the fissure is healing. Best of luck.
