Pregnancy : Pregnancy is a possibility, and you are right that if you take antibiotics that it can cause your birth control pills to not be effective. Please take a urine pregnancy test to put your mind at rest. I am concerned about your back pain--if you have any symptoms of a urinary infection please get that tested, these include going more often, burning with urination, back pain, vomiting, and fever. If your symptoms persist or worsen then please have a medical evaluation.

Possibly: Many women experience breast tenderness and some pelvic discomfort as part of premenstrual symptoms. If your period is not due yet for a few days or a week, a home pregnancy test may not be accurate now and if negative should be repeated a week after a missed period. If there is a fever or pain with urination, a pelvic infection or UTI is possible and needs assessment.
