Regardless : Regardless of wheter or not the doctor believed you, it is a part of the history you gave and should be documented in the chart. It can be documented as a reported fall. The doctor has an obligation to document in the notes if he/she feels the injury does not coincide with the exam, and how this conclusion was supported. If you do not agree with the doctor, you can always seek a second opinion. How do you know that it was not put in your file? Did you request a copy of the emergency room records?
