Many things: Epidural steroid injections work best with pain that radiates down one or both legs from a disc bulge. Back pain by itself or muscle spasms don't respond to the epidural as well. Your pain may be coming from facet joints, sacro-iliac joints or many other causes. Talk to your doctor about where she thinks the pain is coming from and what other options there may be for treatment.
