Depends on situation: Primary menstrual delay occurs when puberty hasn't progressed to that point, often 2.5 yrs after breast buds.Early cycles can be quite irregular due to lack of ovulation with each cycle. After maturity, they can remain irregular due to polycystic ovaries or pregnancy. Lack of signs of puberty @ 13 or menses by 15 deserves a workup. Some pcos patients only cycle 2-3x/yr.
