Clinical: The signs the a clinician would see with multiples are a uterus that is enlarged beyond dates, symptoms of pregnancy that are stronger than average, and signs of diabetes or high blood pressure. Ultrasound is the only reliable way to detect twins.

Ultrasound: The only reliable way to determine a multiple pregnancy (before birth, that is) is by ultrasound. Blood levels of HCG might be helpful, but are not reliable enough. No symptom is consistent or reliable enough to draw conclusions.
