Many things: Sperm production is inhibited by high temperature (sauna, lap top on thighs), stress, varicoceles, and testosterone supplements (by indirect suppression of testicular function). Some men have genetic mutation in the y chromosome that impairs sperm production. Sperm motility is another critical factor in determining male fertility. A semen analysis can check both sperm count and motility.

Many causes: Lifestyle; smoking and xs alcohol consuption. Anatomic: varicoceles, bilateral testicular maldescent - more when higher & subject corrected later, blocked ejaculatory duct/s. Daily and prolonged use of hot tubs and spandex or tight underwaer. Genetic: klinefelter's syndrome & other chromasomomal trisomies. Inflammation: mumps orchitis & epididymitis. Hormonal & androgen use/abuse. Increasing age.
