Likely muscular: Atypical cardiogenic chest pain needs to be ruled out but it is not likely. Because it is coming and going, it is likely muscular but can also be costochondritis (inflammatory pain where the rib meets the sternum). Other considerations but less likely include shingles and post herpetic neuralgia (residual pain after shingles) and a pinched nerve in your upper back (thoracic radiculopathy)-.
