Colonizing your mouth with a different flora may help. Do some research and see if a dentist well versed in this area can help.

Fungal infection : Your child has a yeast/candida issue that must be resolved first. Taking an antibiotic like metro idazole will not do it and i'm fact may get worse since fungus is controlled by the normal flora. Fix this problem through a candida cleansing diet aka low carbs and your symptoms of a white tongue and bad breath will disappear.
