Infection: Millions of bacteria reside in the large intestine. With rupture, the bacteria leak into the abdominal cavity, creating severe inflammation and potential entry of these bacteria into the bloodstream, leading to sepsis or even death if untreated. Depending on the cause & location of the rupture (and luck), adjacent tissues can potentially "band-aid" the whole; other times, surgery is necessary.
