Aggressive therapy: For a prosthetic (artificial) knee that forms scar tissue and has required manipulation, you must do aggressive physical therapy to get motion back. This type of therapy is often painful, and should be accompanied by daily (sometimes 2-3 times per day) stretching and range of motion exercises. If you avoid the painful bending or painful straightening, you may never fully get motion back.

Rule out infection: A total knee replacement that is not allowing restoration of range of motion, especially after two manipulations, raises concerns about possible infection. A clinical work-up, including lab tests and knee aspiration, is indicated to evaluate for possible infection. A second opinion with a joint replacement specialist may help to identify other potential causes of the knee stiffness.
