Many possibilities: Distraction or preoccupation, discomfort with partner, guilt, feeling physically or emotionally unsafe, self-consciousness, partner finishes too soon, dryness/lack of arousal, lack of sufficient stimulation (eg, clitoral), physical illness or pain, etc. Also, some women "learn" how to climax with their partner over time, some need stimulation other than intercourse, and some just can't.
