See a doctor: With regards to the swelling of the feet as there can be a few medical concerns including something like high blood pressure or heart problems. With regards to the smoking - just stop. You said you did it before, so do it again. It has no redeeming qualities, and is just bad for you.

Possibly-see doctor: Swollen feet occur for variety of reasons. These include: chf, chronic venous insufficiency, chronic lymphedema, for pulmonale, right heart failure, ivc syndrome. The feet, being more dependent than other areas of the body can retain fluid in the tissues by gravity driven effects (partic in patients with chronic venous insufficiency).
