Sex change reversal?: Transgender patients undergo hormonal and surgical reassignment medications/surgeries. Medications can be stopped, but surgery cannot be "undone." gender reassignment is not a "sex change operation" anyway; this is a complex set of medical/surgical procedures designed to help transgender individuals match their physical appearance and hormonal status with their proper gender, not the one born with.

Gender reassignment: Yes, in theory, it is possible to surgically reconstruct the organs which define gender, such as breasts, penis and testicles, however the cost may be prohibitive, especially if the patient is evaluated as a "failure" of pre-op evaluation for gender reassignment. Prior to sex change ops, candidates are evaluated by multi-specialty disciplines, to ensure that their gender dysphoria is authentic.
