It depends: In a early pregnancy, a test might be negative because it is done too early (before the time that HCG becomes detectable). A positive test usually means a definite pregnancy. So if the test becomes positive, it usually means pregnancy. If there is a positive test, followed by a negative test, particularly if there is cramping or bleeding, it could mean that there is a miscarriage.
