Early pregnancy: Irregular or long cycles and an unexpectedly early appearing pregnancy should be monitored with u/s. A small gestational sac without a visible embryo could be just earlier than thought. In 2 weeks there should be a viable embryo seen.

8 weeks pregnant: If you are sure of the date of your wife's lastvmenstrual period, there should be more than just a sac at 8 weeks. You should have hormones drawn, or it may be that you are a month off, even then at four weeks a heart beat is often already seen.
