Complicated: I would look for other causes, although you might have compressive symptoms from your thyroid gland, i would make sure it's not something simple like reflux before taking out your gland. You need to be worked up properly. But it very well could be your thyroid.

Need to examine: It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests. You may need a fine needle aspirate of biopsy to settle the issue.
