Depends: Bleeding could be bad or good sign. Bleeding means blood is getting to area and might mean it's healing. But could mean infection. Look to see if pus is present. Look at edges of sore to see if red (good) or white (bad). Tip: stem cells from your fat are now being injected around the sore and the results are promising, especially with difficult ulcers which don't heal.

It is getting deeper: If bleeding, the wound is now at least eroding into the skin itself, indicating at least a stage 2 pressure ulcer (bed sore). Stage 1 is just red skin. Stage 3 is through skin and 4 is down into muscle or bone. You need to be very careful to avoid sheer and moisture, as well as to avoid any pressure on the area to allow it to heal. Stage 3-4 often require complicated surgeries -- avoid these!

Considerations: The real question to ask is...Is the bed sore getting better? Are the dimensions decreasing, is the patient feeling better, is there less "non-healthy" tissue, is there more granulation tissue (new capillaries) ? Bleeding can occur even with great healing as new capillaries get formed and injured with dressing changes or simple movement. If the wound looks and measures better, all else is trivia.
