Tubes tied pregnant?: The answer is yes. It is possible. In about 1/400 cases the tubal ligation procedure experiences technical failure. Not very likely, but possible. As far as pregancy goes, in case of doubt, get a test.

Yes: Nothing, other than complete abstinence, is 100% perfect at preventing pregnancy. A woman with a tubal ligation in her 20's can expect a pregnancy rate around 1:300, in her 30's 1:200. Likely your irregular bleeding isn't due to pregnancy, though an at home pregnancy test is an easy way to be sure.
