Not good: You must see your obgyn asap.It can be one of the symptoms of breast cancer.There can be a few benign things as well.But any blood stained nipple discharge needs to be evaluated asap.

Ductal papilloma. : This is a small growth that can grow in a breast duct. You should see a breast surgeon or specialist, a ductogram can be done to find the papilloma which should be removed, there is a chance it could become cancerous.
