Not likely: Condoms when used correctly at every effective. Make sure there is no contact between the penis and vagina without the condom. Use of an over the counter spermicide can decrease risk as well.

Highly unlikely: Would be most unlikely, but remember condoms and withdrawal are not 100% foolproof. Failure to have a male organsm also doesn;t mean some sperm are not released.
