Uterus/Bladder/Bowel: There are three main structures in the lower abdomen. Infection in them can hurt . Labor can also--even at 20 weeks. There is no way to know what is causing pain without an exam by a doctor. Because of the pregnancy, it is important for a woman to call OB or go directly to the hospital. An evaluation might include blood and urine tests as well as ultrasound and pelvic exam for labor, uti, etc.
