Why do i get a sharp lower back pain when laying in bed? it then subsides to a dull ache. same pain when standing for long periods.

Structural issue: This most likely means there is something structurally affected in your back which most likely sounds like a facet joint issue by how the pain is brought on. If it feels worse when present arching your back and better bending forward it could be this issue. If it is affecting your sleep or daily routine get it evaluated.

Likely muscular: Sounds like muscle pain. In young patients this can indicate conditions such as transitional vertebra or spondylolysis. Other causes include ovarian cysts, disc herniation etc. Majority of folks with symptoms you describe will get better over time (without any specific treatments). Ibuprofen (motrin/advil etc) 400mg every 8 hours for 5-7 days may help. Have you tried a firm mattress?
