Genetic accident.: Trisomy 18 is a generally perinatally lethal (few exceptions prove the rule) genetic aberration that occurs when chromosomal non-disjunction occurs in either the maternal or paternal gamete (egg, sperm). There is no cure and comfort care is all that is ethically permissible.

Error in formation: During the formation of the egg or sperm, the chromosome(c) pairs split with one of each going to the egg or sperm. At conception, the egg & sperm each bring 1 of each (c) & the embryo starts with a pair of each.If there is an error & the 18pair doesn't split but donates a pair of 18's to the egg, then it picks up a 3rd from the sperm. Age/toxins/radiation etc all may play a role in (c) errors.
