Home health: Home health provider is usually a company that provides medical services in the home. Physical therapy and nursing care are the most common. A person who provides the care is either a registered nurse, physical therapist or health aid.

Home health care: Agree with Dr. DAmico. Home health care services include: skilled nursing, wound care, giving medications, insertions/ removal of tubes, giving injections, drawing blood, urine, etc for lab testing, providing different types of therapy (like physical therapy/ speech therapy, rehab), monitoring one's health & much more.
