It doesn't: There is no evidence that testosterone gel affects penile growth or enlargement if used after puberty. Used as a replacement for low blood testosterone to restore libido and also quality of erections. It increases penile blood flow, thus penis may appear larger, plus erections may last longer. It will not make a penis larger.
