Yes: The splinter should be removed, antibiotic may be needed to prevent infection. A booster dose of tetanus vaccine is also in order.

Depends: If you can coax it out yourself, clean it well and he is up to date on shots you might be able to monitor it at home.Sometimes it takes using a local anesthetic for the kid to remain still and extract the splinter. If it has has time to get infected, a coarse of antibiotics may bee needed.
