Multiple causes.: Psychological causes are common, especially if there is anxiety, or any thoughts or emotions that disrupt the cycle of arousal and stimulation. Physical causes include erectile dysfunction with age, diabetes, and atherosclerosis. One cause that is a medical emergency is penile fracture, where the fascial sheath of the erect penis tears during vigorous sex. This needs to be corrected surgically.

Many things: Including emotional and/or physical fatigue. Also sometimes men need quite varied physical stimulation to maintain an erection during intercourse.
