Sometimes: In the event it is used when someone has coronary artery disease of cerebrovascular disease and they can't handle the increased blood pressure it causes, a stroke or heart attack is possible. Hopefully it isn't used in this circumstance unless there is no other choice.

Maybe: There is too much of a good thing. Epinephrine is a natural hormone with necessary functions. Too much can, and does, cause heart attack, hypertension, anxiety, stroke. Aside from exogenous sources, few natural conditions cause excess Epinephrine (eg, pheochromocytoma).
